PeRC: A Beacon of Collaboration
The Pediatric Resilience Collaborative (PeRC) in Santa Barbara County permits healthcare providers to refer clients to community-based organizations (CBOs) directly through FindHelp.
That means CBOs that use VerticalChange and participate in PeRC can now send and receive referrals through FindHelp, via a data integration.
The integration automatically imports referrals from FindHelp into VerticalChange, where custom views are used to match referrals to existing CBO clients. Client and referral data from FindHelp will generate (or update) client profiles in VerticalChange, allowing agencies to continue their invaluable work.
Enhancing Transparency and Closing the Referral Loop
The integration between VerticalChange and FindHelp goes beyond simple one-way data transfer. Status changes made in VerticalChange to imported referrals are automatically pushed to FindHelp. This closes the referral loop and keeps providers informed about referral success rates. The increased transparency nurtures collaboration and provides clarity on outcomes.
CBOs using VerticalChange can also create referrals, which are automatically sent to FindHelp. Referrals generated in VerticalChange will show on an organization's FindHelp inbound referral dashboard. Suppose a FindHelp user updates a referral status in a VerticalChange-generated referral. In that case, the status is also instantly updated in the VerticalChange record to keep everyone up-to-date.
